Our patients are our top priority.

We believe that building relationships with our clients and other service providers is the best way to foster a strong and healthy community. For more than 40 years, SMHC has provided comprehensive quality healthcare to individuals and families in our community. We deliver care with sensitivity, respect and compassion, and believe that helping our patients feel welcome and comfortable is a critical first step towards the trusting relationships that enable consistent care.

Our approach to delivering quality healthcare — combined with the vital services provided by our network of private and public partners — helps patients choose and maximize their health priorities.

Need payment assistance?

Patients who do not qualify for Healthy San Francisco, Medi-Cal, Medicare, or Family PACT may qualify for the sliding fee discount program. SMHC established a sliding fee discount program to provide essential health care services for patients who cannot afford care due to low-income, lack of sufficient insurance or do not qualify for any other program. It allows patients to pay a reduced fee to cover basic comprehensive health services. Discounts are offered based on family size and income. Please bring your proof of income with you to the visit to speak with an Eligibility Specialist.
